Become a part of Parkdale Queen West Community Health Centre as a Registered Nurse for a full-time contract. Play a vital role in delivering community health services and addressing barriers to care while working passionately within a supportive team.
This position focuses on providing a range of client-centered services, emphasizing health promotion and disease prevention. Your varied responsibilities include patient assessments, immunization administration, and the development of chronic disease management programs. You'll also facilitate access to services for the community you serve.
Key Responsibilities:
• Assess patient needs and manage triage
• Track medical follow-ups for health screenings
• Conduct immunizations and allergy injections
• Perform clinical procedures and health assessments
• Participate in outreach nursing to augment service access
Requirements:
• Bachelor of Science in Nursing or equivalent
• Registered with the College of Nurses of Ontario
• 3+ years in community or public health nursing
• Experience in primary care settings is an asset
• Proficient in electronic medical records usage
